Insurance and Payment
***Insurance:
Mindfully Integrated Health accepts the following insurance plans for most services, including medication management and therapy:

Please refer to office policies document on our FORMS page for complete financial and office policies.
Currently we are Out-of-Network for all other insurance providers. You can check with your insurance for out-of-network benefits.
We do not currently accept any United Health Care plans at this time
You may request a super bill to submit to your insurance provider for reimbursement of any covered out-of-network expenses.
As always, we offer a private pay option for all patients as outlined below.
Payment:
Payment of charges is due at the time of service, including all deductible, copays or coinsurances as outlined by your insurance carrier.
We gladly accept cash, check or debit/credit card.

Private pay rates:
Dr. Wicklund - New Patient Evaluation: $350
Dr. Wicklund - Follow up/brief therapy: $200
PA Wright/Monette- New Patient Evaluation: $350
PA Wright/Monette- Follow up/brief therapy: $175
Individual Therapy (Licensed therapist): $150
Ketamine infusion therapy (per infusion):
With medical provider (private pay) $500
With medical provider (billed to insurance) $200 + copay
Insurance billing for ketamine are subject to applicable copay/coinsurance requirements
Listed fee is patient responsibility
Ketamine Assisted Psychotherapy (per session):

Financial Policies
Please read the following clinic policies as they apply to all patients:
All patients utilizing insurance plans must provide insurance information at the time you schedule your first visit.
Insurance benefits will be verified by MIH staff prior to your visit.
All copayments and coinsurances are due at the time of service. No outstanding balances will be allowed.
Release of records and/or prescriptions will be held until all balances are paid in full.
If you need to cancel an appointment, you must contact the clinic not less than 24 hours prior to your scheduled appointment.
Cancellation within 24 hours of an appointment or a no show appointment will result in a $75 no show fee. This will be charged to your account and must be paid prior to scheduling of a new appointment.
For therapy appointments, after the second no-show/late cancellation, each subsequent no-show/late cancellation may result in a full charge of the private pay rate ($200 for psychologist/$150 for therapists).
If a patient shows a consistent pattern of late cancellation or no-show (3 or more) we reserve the right to discharge the patient from therapy for non-compliance.
We respect the time you are taking to address your mental health needs and ask that you respect our time as well. In order to provide you with the time necessary to provide quality care, we ask that you please have all new patient questionnaires and paperwork filled out PRIOR to arriving for your first visit.
MIH staff will make every effort to contact you regarding your incomplete intake paperwork but reserves the right to cancel any appoint in which the questionnaire has not been completed as of the day of the appointment.
Any appointment cancelled due to incomplete paperwork will result in the rescheduling of your appointment and a $75 rescheduling fee may be charged.
